Filgrastim, sold under the brand name Grastim among others, is a medication used to treat low neutrophil count. Low neutrophil counts may occur with HIV/AIDS, following chemotherapy or radiation poisoning, or be of an unknown cause. It may also be used to increase white blood cells for gathering during leukapheresis.

Filgrastim injection is used to treat neutropenia (low white blood cells) that is caused by cancer medicines. It is a synthetic (man-made) form of a substance that is naturally produced in your body called a colony stimulating factor. Filgrastim helps the bone marrow to make new white blood cells

Filgrastim may be given subcutaneous (the layer between the skin and muscle) injection or infused into a vein (intravenous, IV). Filgrastim is generally given on a daily basis. The number of days you receive filgrastim will be prescribed by your doctor. Filgrastim should be refrigerated.

Common side effects may include:

    fever, cough, trouble breathing;
    nosebleeds;
    bone pain, muscle or joint pain;
    diarrhea;
    headache;
    numbness; or.
    rash, thinning hair.

Patients receiving chemotherapy or who have received a bone marrow or stem cell transplant are only required to use Filgrastim for short periods of time until the number of infection-fighting neutrophils are restored (usually 1 to 3 weeks). Stem cell donors should receive Filgrastim treatment for 4 to 5 days.

Mechanism of action

Filgrastim is a human granulocyte colony stimulating factor (G-CSF) produced by recombinant DNA technology. G-CSF regulates the production of neutrophils within the bone marrow; endogenous G-CSF is a glycoprotein produced by monocytes, fibroblasts, and endothelial cells.[medical citation needed]

G-CSF is a colony stimulating factor which has been shown to have minimal direct in vivo or in vitro effects on the production of other haematopoietic cell types. Neupogen (filgrastim) is the name for recombinant methionyl human granulocyte colony stimulating factor (r-metHuG-CSF).

Hepcinat Tablet is an antiviral medication. It is used along with other medications for the treatment of chronic hepatitis C virus (HCV) infection. It works by lowering a load of hepatitis C virus in the body and removing the virus from the blood over a period of time.

Hepcinat Tablet should be taken in the dose and duration as advised by your doctor. Do not skip any doses and finish the full course of treatment even if you feel better. It should be taken with food, preferably at a fixed time. If you miss a dose, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and go back to your regular schedule. Do not double the dose.

It has fewer side effects as compared to other similar medicines, like fatigue, headache, nausea, insomnia (difficulty sleeping) and anemia (low red blood cell count). How to cope with side effects?The occurrence of side effects varies from person to person. The following are a few ways of dealing with some of the common side effects. However, consult your doctor if these persist.
  • Coping with NauseaYou can help yourself by eating small, frequent meals rather than large ones and drinking plenty of fluids. Eat slowly. Avoid fatty, fried, spicy and very sweet foods. Eat cold or slightly warm food if the smell of cooked or cooking food makes you feel sick. Get plenty of fresh air. You could also try chewing ginger or drinking ginger tea. Eat bananas to replace potassium in your blood which can drop if you are sick (vomit). Use oral rehydration salts to replace vitamins and minerals lost through being sick. There are some medicines that can help you stop from feeling sick. Speak to your doctor if your condition does not improve.
  • Coping with HeadacheMake sure you rest and drink plenty of fluids. Rest in a quiet, dimly lit room. Do not sleep more than you normally would. Do not strain your eyes (for example by looking at a screen). Do not drink alcohol. Headaches are usually temporary and usually go away with time. But, if they last longer or get worse, ask your doctor to recommend a painkiller.
  • Coping with Anemia (low number of red blood cells)Making some simple lifestyle changes can raise your red blood cell count. Reduce the amount of alcohol you drink, eat a well-balanced healthy diet and take regular exercise. Physical activities make your body need more oxygen and this stimulates the production of red blood cells. There are some vitamins which are important such as vitamin B-12, vitamin B-9, vitamin C and Vitamin A. Also, some minerals such as iron (especially) and copper. Aim for foods which contain higher quantities of these vitamins and minerals or ask your doctor for supplements. Make sure to consult your doctor for complete evaluation and treatment of anemia.

Pegfilgrastim injection products are used to reduce the chance of infection in people who have certain types of cancer and are receiving chemotherapy medications that may decrease the number of neutrophils (a type of blood cell needed to fight infection)
Pegfilgrastim Is Use For: This medicine is used to stimulate the growth of "healthy" white blood cells in the bone marrow, once chemotherapy is given. White blood cells help the body to fight infection. This is not a chemotherapy drug.
Neutrophils are short-lived immune cells that are highly susceptible to cell death following myelosuppressive chemotherapy. This marked reduction in neutrophil numbers during chemotherapy increases the risk of hospitalization, infection, and infection-related mortality. It also directly impacts the clinical outcome of patients if cases of febrile neutropenia requires dose reductions or schedule delay of chemotherapy, thus reducing the clinical efficacy of chemotherapy and patient benefit from receiving appropriate treatment

G-CSF is an endogenous haematopoietic growth factor that stimulates granulopoietic cells of the neutrophil lineage. Pegfilgrastim mimics its biological actions and binds to the same G-CSF receptor expressed on cells of myeloid lineage, such as granulocytic precursors and mature neutrophils.Upon binding of the ligand, G-CSF receptor undergoes a conformational change and activates several downstream signalling pathways including JAK/STAT, PI3K/AKT and MAPK/ERK. These pathways work to increase proliferation and differentiation of granulocyte progenitor cells, induce maturation of the progenitor cells, and enhance survival and function of mature neutrophils.
